Choosing a single country out of hundreds did not resonate with the creators as the survey overtly claims it is about the future of creators. Creators come from multiple countries. You are right; the implementors of the study shouldn't have posted this to everyone and disappoint them. I still don't understand the point of a $5 Amazon voucher for 45 minutes of effort. You also raised an important point that Medium may take subsequent follow-ups for considering other countries and age group samples; thus, I provided this valuable feedback as a lesson learned.
